Who Can Join:

You can join HealthCare Connect if:

• You do not qualify for any other programs (Medicaid, Medicare, employer-based commercial or private). Eligibility for AHCCCS or other government funded healthcare programs will be determined prior to enrollment in HealthCare Connect.
• You live in Maricopa County.
• Your income is between the levels listed in the chart below.

Each year the Federal Poverty Level is adjusted by the Department of Health and Human Services.

How to Join:

It's easy to join HealthCare Connect. Members pay their enrollment fee and then choose a primary care physician who oversees all of their care, ensuring complete, coordinated services. Members also pay very low fees at the time they receive medical services. Participating physicians, hospitals, facilities and health centers that are committed to caring for uninsured Maricopa County residents provide these medical services.

Membership is effective immediately upon verification of eligibility and receipt of enrollment fees. Enrollment fees are non-refundable.
