What Can HealthCare Connect do for Our Community?

• Establish a medical home for Maricopa County’s low income by helping each member find a primary care home to oversee his or her total health care.
• Provide a contracted network of primary care, specialty care and hospital facilities of sufficient scale to meet the needs of the members.
• Screen and enroll eligible applicants for available public insurance programs, utilizing a web-based electronic eligibility-screening tool called Health-e-Arizona. This will help place Maricopa residents who qualify into available programs such as Medicaid, KidsCare, and place others into the health care network of HealthCare Connect.
• Provide a network of community based pharmacies to offer discounted prescriptions.
• Standardize information systems for eligibility, enrollment and quality management.
