HealthCare Connect is a non-profit organization that connects low-income, uninsured residents of Maricopa County with complete, coordinated health care at affordable rates. Healthcare Connect is a medical discount program.

HealthCare Connect is a powerful private-public partnership of government, hospitals, physicians, and others in the health care industry working together to make Maricopa County a healthier community.

HealthCare Connect is affordable:

• There are no monthly premiums
• Enrollment fees are $50 a year for individuals;
  $100 a year for families of two or more
• Very low fees are due at each visit
• Enrollment fees are non-refundable.

HealthCare Connect is easy to use:

• You pay only for services used
• No insurance forms
• No medical conditions excluded
