HealthCare Connect is a non-profit organization that connects qualified, uninsured residents of Maricopa County with complete, coordinated health care at affordable rates. HealthCare Connect is a medical discount program.

HealthCare Connect was established to help make Maricopa County a healthier community in partnership with the health care community, government and private agencies.

HealthCare Connect is affordable:

• There are no monthly premiums
• Enrollment fees are $50 a year for individuals;
  $100 a year for families of two or more
• Very low fees are due to the health care provider at each visit
• Enrollment fees are non-refundable.

HealthCare Connect is easy to use:

• You pay only for services used
• No insurance forms to be completed
• Eligibility is not denied because of existing medical conditions

You will need to include the following documentation when sending your application:

• Proof of Residency (Such as a utility bill, phone bill, etc.)
• Proof of Income (Pay stubs for the last 30 days)
• Sign the application
